Description

The ExitPlanMode tool in Claude Code does not appear to be working correctly. When invoked, it fails to exit plan mode as expected, requiring manual intervention.

Steps to Reproduce

  1. Enter plan mode during a conversation
  2. Write a plan and attempt to use ExitPlanMode
  3. Observe that plan mode is not exited

Expected Behaviour

ExitPlanMode should transition out of plan mode and present the plan for user approval.

Actual Behaviour

The tool call appears to fail or have no effect, leaving the conversation stuck in plan mode.
